Health Calls uses the Honeywell HomMed Health Monitoring System to support people with chronic diseases at home. This tool is used in conjunction with visits for health education and monitoring. The monitor is very useful in teaching a person with chronic disease (such as high blood pressure, diabetes, heart failure and emphysema or COPD) about the body’s daily response to medication, food, and fluids. The results are used to help a person monitor modifications needed to improve symptoms and overall health.
The HomMed is a hospital-grade medical device that collects vital signs daily, providing patients with a secure and consistent connection to Health Calls clinicians. If information collected indicates a need for intervention, or if the user fails to collect vital signs when prompted, appropriate action is taken and members of the care team may be contacted.
The HomMed Health Monitoring System is easy to use, and takes only three minutes to collect patient vital signs from the comfort of home. At a prescribed time each day, a user-friendly voice guides patients through a step-by-step process to measure weight, blood pressure, pulse, and oxygen saturation.
